Order Contact Lenses

This form is for our existing patients who have previously ordered contact lenses from our practice. If you are not an existing patient, please either book an appointment online or phone us on 3396 1755 to make an initial contact lens consultation. This form will allow us to place a contact lens order for you. Please complete all the fields below.

Terms & conditions:

To place an order, you must have been seen by an optometrist within the last 12 months for a contact lens assessment/review and have a valid contact lens prescription. If it is over 12 months, we will contact you to arrange a review before we are able to place an order.

If delivery requested, our practice staff will phone you to confirm delivery address and arrange payment over the phone before your order is dispatched. Please note, health fund claims can not be processed over the phone so this will need to be done directly with your fund if required.

If in store pick up requested, health fund claim can be processed onsite (if applicable) and payment made on collection.
